Two Rivers Public Health Department underwent a community health assessment process in 2024. In the early months of 2025, TRPHD will embark on a community health improvement planning process with the intent of implementing a new Community Health Improvement Plan.
Data gathered through community health assessment processes are used for strategic planning and community health improvement plans.
